iCreate Summer Camp Registration Form for Children 7-12 yrs old. Camp dates: Aug. 22 - Aug. 26, Full-day camp, 9.00 to 4.00 PM, Fee: $442.50 + HST (13%) = $500 To register please fill out the on-line form below and submit deposit ($50 for half-day, $100 for full-day) by e-transfer to artstudioicreate@gmail.com. The remainder of the fee ($200 for half-day, $400 for full-day) is due no later than 24 hours before the start of the camp. We confirm receiving of the payment and registration by email.

Program Disclaimer	COVID-19 Preventive Measures Masks are mandatory while indoors for all camp participants, parents/guardians dropping off and picking the children, and for all staff members. We recommend the use of good quality masks (e.g.: N95). Registrants are not allowed to attend classes if they exhibit any COVID-19 symptoms. Furthermore, they need to follow Ontario Public Health isolation procedure and complete a recovery process before returning to class. Parents or guardians of our program registrants must inform iCreate staff immediately about any COVID-19 symptoms their child may have developed during the course of our program, or within 10 days of its ending. Parents and guardians must conduct a daily check for COVID-19 symptoms for each child participating in our programs prior to bringing the child to the class. Selected list of COVID-19 symptoms: Fever (temperature of 37.8℃ or greater) or chills, Cough, Shortness of breath, Decreased or loss of taste or smell, Runny nose or nasal congestion, Headache, Extreme fatigue, Sore throat, Muscle aches or joint pain, Gastrointestinal symptoms (such as vomiting or diarrhea). Payments, cancellations, refunds The non-refundable deposit ($50 for half-day camp, $100 for full-day camp) is required at the time of registration. A full camp payment is required no later than 24 hours before the start of the camp. If a camp cancellation was made following a submission of the full camp fee a refund of the camp‘s fee less the deposit will be issued only if the cancellation was received and confirmed by iCreate art studio staff at least 24 hours before the start of the camp. Cancellations made less than 24 hours before the start of the camp, on the camp‘s starting date, or any time during the camp‘s period are not eligible for a refund of any part of the camp‘s fee. We are unable to refund, reschedule or credit any days missed during the camp. Outdoor breaks and field trips I give a permission for my child‘s participation in outdoor breaks and field trips. I acknowledge the full application of the ‘Safety waiver‘ provisions to the outdoor breaks and field trips settings. Safety waiver Registrants` safety is the iCreate`s staff highest priority. However, I acknowledge and agree with that iCreate owners, landlords, agents and staff are not responsible for injuries, damages or losses I or participants I have registered for iCreate programs may sustain while participating in iCreate programs or related activities, such as recesses, field trips, personal breaks, participants` drop-offs and pick-ups and other. I hereby accept full responsibility for any injuries, damages or losses I or my child may sustain while participating in iCreate programs or related activities. In case of emergency In case of an emergency I authorize iCreate staff to arrange any necessary medical treatment from a licensed health care provider for me or the program participant(s) I have registered for iCreate program(s).
