Registration Form for NEW REGISTRANTS: Drawing and Painting for Children 7-10 years. Program time: Wed., 4.30 to 6.00 PM, Age: 7-10 yrs, Cost per class: $35.39 + HST (13%) = $40.00 To register please fill out and submit the on-line form below. To complete the registration please submit the payment (cash, cheque or e-transfer). Cash and cheques (payable to iCreate) can be submitted to our studio location at 987 Clarkson Rd. S. L5J 2V8, Unit 102, Misssissauga, ON. E-transfer can be sent to info@icreate.ca. We confirm receiving of the payment and registration by phone or email.

Program Disclaimer	iCreate Art Studio - Registration Agreement and Health Measures By registering, I agree to comply with all iCreate studio's Registration and Health Measures. I understand that refunds/credits will not be issued for missed classes due to non-compliance with Registration and Health protocols, even if such protocols change during the session. Health & Studio Access: Masks are optional. Participants showing respiratory symptoms may be asked to wear a mask and wait for pick up in the designated area. Only participants and staff may enter the studio; parents/guardians must remain in the hallway. Participants must not attend if experiencing symptoms such as fever, cough, runny nose, sore throat, fatigue, or gastrointestinal issues. Return is permitted only after full recovery. Cancellations & Refunds: Cancellations are accepted up to 7 days before the start date (full refund). Less than 7 days' notice: 25% admin fee applies. Registration is confirmed only upon full payment. No refunds for missed or dropped classes after the session begins. Rescheduling: One (1) class may be rescheduled in a session of 5-10 classes. No rescheduling is permitted for sessions with fewer than 5 classes. Rescheduling request must be communicated to and confirmed by iCreate staff at least 1 hour before class to be eligible. No-shows without notice, or with a notification less than 1 hour before the class will not be rescheduled. Art Projects: All participants work on the same project. Projects not completed due to missed classes or a slower work pace must be finished at home. Liability Waiver: I understand that iCreate and its staff are not liable for injuries, damages, or losses occurring during programs or related activities. I accept full responsibility for myself or my child. Emergency Consent: I authorize iCreate staff to seek medical treatment in case of emergency.
